Boone Memorial’s Swing Bed Program is designed for short-term rehabilitation after an acute hospital stay. The goal is to help patients quickly reach their optimal level of independence after injury, illness or surgery and return safely home.

Swing Bed is a concept developed by Medicare in the 1970’s to help with underutilized hospital beds and the lack of nursing home beds in rural areas. This program is available to patients of BMH and patients whose initial acute stay may have been at another facility.

The Benefits to this Program are:

  • Allow patients to stay close to family and friends

  • Specialized care/services

  • An individualized treatment plan tailored to the patient’s personal situation

  • Experience all the advantages of a hospital-based rehabilitation program

  • Have an exceptional team of nurses, activity coordinators, nutrition experts, pharmacists, social workers, and physical therapists, all working together to make sure the patient’s health goals are met

How do I qualify for Swing Bed under the Medicare Program?
